Personal Data
Internal Affairs handles your personal data with care and confidentiality. As of 25 May 2018, this has been regulated under the General Data Protection Regulation (GDPR).
If you make use of services from Internal Affairs, I will require certain information from you. You may provide this via telephone (for example, when scheduling an appointment), through the contact form on www.internalaffairs.nu, or by email.
This may include:
• Name
• Date of birth
• Address details
• Telephone number
• Email address
• Certain relevant (medical) information
Why is this necessary?
Internal Affairs needs your personal data to carry out its services, provide you with personalised advice, and support you during guidance or treatment. In some cases, this may involve “special” and/or “sensitive personal data.” These will only be collected when necessary for your treatment or guidance.
Your data will not be used for commercial purposes and will be stored for 20 years in a secure online environment.
Sharing with third parties
Internal Affairs will only share your data with other healthcare providers if necessary and only with your explicit consent (for example, reporting to a referring doctor or transferring records to another institution in case of admission).
Internal Affairs may provide your data to an ICT hosting organisation and an electronic client file system for secure storage. Agreements are in place with these organisations to ensure standard levels of security and confidentiality for your data. Internal Affairs remains responsible for the processing of your personal information.
Viewing, modifying or deleting your data
You have the right to view, correct, or request deletion of your personal data. To do so, you can send a request to nathalie@internalaffairs.nu. Because identification is required, a personal appointment will be scheduled.
Data protection
Internal Affairs takes the protection of your data seriously and takes appropriate measures to prevent misuse, loss, unauthorised access, unwanted disclosure, and improper alterations.
